• Pre-Arranged Absence Forms If your student is planning to be out and you know in advance, please use the appropriate form for your student's grade level to arrange for the absence. Students should have each of their teachers write down what they will miss in class during the absence and then have it approved in the front office before the pre-arranged absence will be accepted as excused.

  • Lower School (K5 - 6) Pre-Arranged Absence Form 

  • Middle School (7 - 8) Pre-Arranged Absence Form

  • High School (9 - 12) Pre-Arranged Absence Form

 

• Immunization Schedule The required immunization schedule lets parents know at a glance what immunizations their child may need to schedule.

 

• Community Service Hours Log FormBeginning with the class of 2028, all students must have 100 community service hours as a graduation requirement. These hours are also required for Florida Bright Futures scholarships. Please use this form to document work performed.  See the guidance counselor for more information. 

 

• Consent and Release FormThis Consent and Release Form is required for every field trip or off-campus event.  If you need a permission slip, please print this, fill it out, and turn it in to the proper person.

 

• Educational Records Release Form 

Please sign and turn in this form with your application to Aletheia.  This form allows us to receive your student's educational records from his/her previous school.

 

• Pastor's Reference Form 
   This pastor's reference form is required for all new applicant families.

 

• Withdrawal Form

  This form is used to apply for withdrawal of your student from Aletheia Christian Academy during a school year.
