  • Parents will need to fill out and return this AOR for the 2021-2022 school year.

  • If you wish for your high school student(s) to attend our after school care program because you are unable to pick them up at 3:00pm, please fill out one form for each student and turn it into the office. There is a $10 annual registration fee PER FAMILY for after school care services, plus hourly usage charges.

  • If you wish for your elementary child(ren) to be able to access our After School Care program in the event that you are unable to pick your child up by 3pm, please fill out a form for each child and turn that in with a $10 annual registration fee PER FAMILY.  Thank you!

  • All parents willing to help by being a volunteer driver for field trips or other special events need to fill out this form annually.

  • All students using Aletheia computers, iPads, other devices, or the internet at Aletheia and their parents are required to sign this agreement.

  • This pastor's reference form is required for all new applicant families.

  • A photo release form should be on file for every student at Aletheia.  If you do not wish to give permission for your child's image to be used for our school's purposes (other than the school yearbook), please note that fact on the form, sign it, and return it.

  • If your elementary student is planning to be absent, please fill out this form and have the teacher write down the work that will be missed during the absence.  Once signed, this should be returned to you so that you can ensure all needed work is made up upon return to school.  Thank you!

  • If your high school student is planning to be out and you know in advance, please use this form to arrange for the absence.  Students should have each of their teachers write down what they will miss in class during the absence and then have it approved in the front office before the pre-arranged absence will be accepted as excused.

  • Parents and students wishing to order spirit shirts need to fill out this form and turn it in with payment by Friday, August 20, 2021. Students may wear their spirit shirts with uniform bottoms every Friday of the year. They may also be worn in lieu of PE uniform shirts and be worn to all ACA events to show your school spirit!

  • This is Aletheia's VPK Registration form, which is required for all students enrolling in our VPK program.  Please note that this form is NOT a substitute for the VPK application which needs to be filled out with the Escambia County Early Learning Coalition.

  • Parents enrolling their child in Aletheia's VPK program need to sign and turn in this form.

  • The VPK Program is free, and is offered M-Th from 8am-12pm.  There are optional services that you can request that have fees associated with them.  Please check this list here.

  • This form is used to apply for withdrawal of your student from Aletheia Christian Academy during a school year.
